Please fill out this form completely to be considered for a TFP/TFCD
or a photography project.
To be
considered, you must

Be at least 18
years of age;

Have a
defined, muscular, athletic or lean build;

Be comfortable with yourself;
- Be willing to
travel to the Tampa Bay area, or another agreed upon location;
- Review the
Preparation Guide sent to your E-mail;
- Schedule your
consultation, if local;
- Be punctual to your appointments, with 48
hour advanced cancellation notification.
Images submitted for review must be NEW/ORIGINAL,
not images posted on your social media.
Body shots do NOT need to be nudes; ideally you should be in fitted
underwear, such as briefs or boxer briefs, showing shoulder to knee,
front and rear.
You may elaborate on any section in
the "Comments" area at the end.
